Kidney Education

The kidneys perform many important functions in your body, including removing waste products and extra water, regulating blood pressure, making red blood cells, and keeping your bones healthy. Chronic kidney disease occurs when kidneys are damaged and don’t work the way they are supposed to. This typically occurs gradually over time.

DCL’s Kidney Care Management (KCM) program will help you learn about CKD and support you through all stages of kidney disease before dialysis. KCM is an interdisciplinary program with a nurse, dietitian, and social worker who will help you take proactive steps to slow down the progression of your kidney disease. However, if your kidney disease does progress to the point, you are getting close to needing dialysis, our program will help you be prepared to make that transition.
