For over 35 years, Dialysis Center of Lincoln, Inc, (DCL) has provided high quality care to individuals needing dialysis as well as educational classes designed to prepare individuals with chronic kidney disease (CKD) for a transition to dialysis. We understand how challenging a diagnosis of CKD is for individuals and their families. We also know that reaching individuals early in their disease process can improve their health outcomes and reduce unnecessary hospital visits.
DCL is excited to announce the launch of our expanded kidney care education program. In addition to bi-monthly group educational classes, we now offer customized support for individuals with CKD. The DCL Kidney Care Management (KCM) program is designed to provide specialized care coordination and support for individuals with stage 3, 4, and 5 CKD. KCM will allow DCL to provide comprehensive kidney care support to individuals with CKD; services that haven’t been widely available in our community until now.
The KCM program began enrolling CKD participants in November with our first clinic held on December 5th. During each clinic visit, enrolled participants will meet with our interdisciplinary KCM team (Registered Nurse, Registered Dietitian, and Licensed Clinical Social Worker.) The team will provide comprehensive care that addresses the unique needs of each individual. This includes disease stage education, personalized care plans, medication management, nutrition counseling, and other interventions to slow the progression of CKD and decrease costs of healthcare utilization.
